Comparison of transcatheter and surgical treatment of paravalvular leak: Results from a 5-year follow-up study

Zhang Y, Pan X, Qu X, et al. - Researchers compared transcatheter repair for paravalvular leak (PVL) with surgical repair in terms of efficacy and safety. Having symptomatic PVL, 87 consecutive patients were treated either using transcatheter (n = 46) or surgical (n = 41) treatment at Shanghai Chest Hospital between January 2009 and December 2015. These were included in this study. The transcatheter group exhibited fewer in-hospital total major adverse events as well as fewer blood transfusions, shorter procedure durations, shorter hospital stays, and less expenditure. Following transcatheter and surgical repair, the reported 5-year overall survival rates were 74.39% and 71.95% and the cardiac-related survival rates were 84.08% and 74.72%, respectively. Findings revealed the efficacy of both transcatheter and surgical repairs for selected patients with PVL. They noted that the safety of transcatheter closure was more and it was more cost-effective. Nonetheless, this novel treatment may be risky for postprocedure hemolysis when unsuitable devices are used.
