Comparison of risk factors for postoperative complications across age groups in patients undergoing ORIF of the ankle

McKissack HM, Viner GC, Jha AJ, et al. - The incidence of and risk factors for adverse postoperative outcomes following open reduction and internal fixation (ORIF) of ankle fractures were examined across patient age groups. Researchers reviewed charts of 886 patients (aged 18 years and older) who underwent ORIF for any closed, non-polytraumatic, non-pilon ankle fracture at a single institution between the years 2008 and 2018. Three hundred seventy-five (42.3%) of 886 patients were over age 50. Findings revealed a significant association of age with an incidence of nonunion. Patients younger than 50 with chronic kidney disease and all patients with CHF and HTN were at significantly increased risk of infection. Tobacco use, substance use, and CHF Risk were significantly associated with wound dehiscence in younger patients. Younger patients with HIV, CHF, and chronic kidney disease, and older patients with HTN were at significantly increased risk of implant failure. Findings support using a multidisciplinary approach for appropriate management of patients with multiple comorbidities.
