Comparison of outcomes of transcatheter vs surgical aortic valve replacement in patients ≥80 years of age

Lemor A, et al. - Among patients ≥80 years who had aortic valve replacement from 2011 to 2015, researchers compared in-hospital outcomes between those who underwent transcatheter aortic valve implantation (TAVI) vs those who had surgical aortic valve replacement (SAVR). Overall 30,590 TAVI and 54,204 SAVR procedures were identified from the National Readmission Database. A total of 19,713 patients remained in each group after propensity score-matching. Lower in-hospital mortality, lower major in-hospital complications, lower 30-day readmission rate, and hospital costs were observed in relation to TAVI, and thus, it was considered an effective and safer alternative to SAVR in the studied age group. Despite this, SAVR continued to be the most common approach in octogenarians, although practice patterns are changing, as suggested by the trends in this data set.
