Comparison of direct oral anticoagulant and vitamin K antagonists on outcomes among elderly and nonelderly trauma patients

Nishimura T, Guyette FX, Naito H, et al. - Given the association of direct oral anticoagulants (DOACs) with a lower risk of bleeding relative to vitamin K antagonists (VKAs), researchers here examined the outcomes of elderly trauma patients receiving DOACs. They screened 32,272 trauma patient. Of these, 530 were taking DOACs and 1,702 were taking VKAs. Six hundred sixty-eight patients were matched in a 1:1 ratio (DOACs group, 334 vs VKAs group, 334). Among patients younger than 65 years, similar mortality was observed in the DOACs group. However, there was reduced in-hospital mortality and reduced administration of fresh frozen plasma among trauma patients 65 years or older taking anticoagulant therapy in correlation with using DOACs vs VKAs.
