Comparison of delay-sensitive and delay-insensitive computed tomography perfusion methods in acute ischemic stroke and their variability according to location of critical vascular stenosis

Karaalioglu B, et al. - Experts intended to assess visual and quantitative differences of delay-sensitive (singular value deconvolution [SVD]) and delay-insensitive (SVD+) computed tomography perfusion (CTP) postprocessing methods in acute ischemic stroke patients and their variability according to the location of the critical stenosis. They found the most prominent change in difference mean transite time among both CTP methods. Visually, in proximal critical stenosis, penumbra was better distinguished by SVD, while it was better distinguished by SVD+ in distal critical stenosis. Penumbra was noted in SVD and pseudohyperperfusion in SVD+, in cases with both ipsilateral critical proximal and distal stenoses. Extraction of contrast delay in the SVD+ method could give rise to false results in cases of ipsilateral critical proximal and distal stenoses.
