Comparison of age (< 75 years vs ≥ 75 years) and platelet reactivity to the risk of thrombotic and bleeding events after successful percutaneous coronary intervention with drug-eluting stents (from the ADAPT-DES study)

Faggioni M, Redfors B, Crowley A, et al. - Researchers used a prospective observational registry, named ADAPT-DES, including 8,582 drug-eluting stents (DES)-treated patients, to examine the link between age and platelet reactivity on 2-year results post-percutaneous coronary intervention with DES. They grouped patients with an age cut-off of 75 years. They performed VerifyNow P2Y12 testing to assess the on-clopidogrel platelet reactivity. They noted a slight rise in residual platelet reactivity on-clopidogrel with age. Greater all-cause mortality, myocardial infarction and clinically relevant bleeding were observed in relation to age ≥75 years, whereas, the risk of stent thrombosis was found to be independent of age. Findings revealed a stronger link of increasing age with the risk of mortality and bleeding than of thrombotic events. The adjusted relative risks of ischemic and bleeding events related to age were not modified by higher residual platelet reactivity despite being connected with older age.
