Comparison of a complete percutaneous vs surgical approach to aortic valve replacement and revascularization in patients at intermediate surgical risk: Results from the randomized SURTAVI trial

Søndergaard L, Popma JJ, Reardon MJ, et al. - In the prospective SURTAVI trial, researchers compared the completely percutaneous approach to aortic valve replacement and revascularization vs the standard surgical approach for patients at intermediate surgical risk with severe aortic stenosis (AS) and coronary artery disease (CAD). Participants were recruited from 87 centers in the United States, Canada, and Europe. Cases with complex coronary artery disease with SYNTAX score > 22 were excluded. Following randomization to treatment, the study sample comprised 169 transcatheter aortic valve replacement (TAVR) and percutaneous coronary intervention (PCI) patients, 163 surgical aortic valve replacement (SAVR) and coronary artery bypass grafting (CABG) patients, 695 TAVR patients, and 633 SAVR patients. In this study, a complete percutaneous approach of TAVR and PCI was identified as a reasonable alternative to SAVR and CABG for patients at intermediate surgical risk with severe AS and non-complex CAD (SYNTAX score ≤ 22).
