Comparing risk stratification criteria for predicting lymphatic dissemination in endometrial cancer

Kilts TP, Glaser GE, Langstraat CL, et al. - Two published risk stratification models (Milwaukee Model vs. Mayo Criteria) were compared regarding their predictive value for lymphatic dissemination (LD) in endometrioid endometrial cancer (EC). Researchers retrospectively reviewed 1103 patients with stage I-III EC undergoing surgery from January 1, 2004–September 30, 2013, and classified them as low-risk vs at-risk for LD using two independent risk models. The analysis revealed a higher accuracy of Mayo Criteria for identifying patients who may safely omit lymph node dissection in endometrial cancer. An unacceptable false negative rate was observed of tumor diameter > 2 cm for predicting lymphatic dissemination. Findings suggest the utility of preoperative endometrial sampling to assist with risk stratification in endometrioid endometrial cancer.
