Comparative effectiveness of medical therapy, percutaneous revascularization, and surgical coronary revascularization in cardiovascular risk subgroups of patients with CKD: A retrospective cohort study of Medicare beneficiaries

Charytan DM, et al. - Including chronic kidney disease patients stratified by cardiovascular disease risk, researchers performed this retrospective cohort study to compare, separately, the risks for kidney failure and mortality following treatment with percutaneous coronary intervention, coronary artery bypass grafting, or optimized medical therapy for coronary disease. Participants were 34,385 individuals with CKD identified from a national Medicare sample. By baseline cardiovascular risk, differences in the outcomes related to cardiovascular treatments were evident. Findings revealed no association of coronary revascularization with improved survival in low-risk patients, whereas it was found to be related to improved survival in high-risk patients despite the finding of a greater rate of kidney failure.
