Communication with older patients with cancer using geriatric assessment: A cluster-randomized clinical trial from the National Cancer Institute Community Oncology Research Program
JAMA Nov 15, 2019
Mohile SG, Epstein RM, Hurria A, et al. - Researchers examined how communication about aging-related concerns are affected when oncologists are provided a summary of geriatric assessment results and geriatric assessment–guided recommendations. They conducted a nationwide cluster-randomized clinical trial enrolling 541 older patients with advanced cancer from 31 community oncology practices within the University of Rochester National Cancer Institute Community Oncology Research Program from October 29, 2014, to April 28, 2017. They performed randomization of oncology practices to receive either a tailored geriatric assessment summary with recommendations for each enrolled patient (intervention) or alerts only for patients meeting criteria for depression or cognitive impairment (usual care). In this study, providing a geriatric assessment summary with recommendations to oncologists was identified to be correlated with an improvement in postvisit patient satisfaction and caregiver satisfaction and an increase in the number of conversations about aging-related concerns. Significantly different results were evident between the intervention and usual care groups.
