Combining qSOFA criteria with initial lactate levels: Improved screening of septic patients for critical illness

Baumann BM, et al. - Researchers examined if improvement in emergency department (ED) screening of septic patients for critical illness could be achieved via adding lactate to Quick Sequential Organ Failure Assessment (qSOFA) scoring. They conducted a multicenter retrospective cohort study of 3743 patients; of these, 2584 had a lactate drawn ≤6 h of ED stay and 18% met the primary outcome. Observations revealed a substantial improvement in sensitivity for the screening of critical illness with the combination of qSOFA ≥1 OR Lactate ≥2 vs isolated lactate or qSOFA thresholds.
