Clinicopathological characteristics of ypT0N0 urothelial carcinoma following neoadjuvant chemotherapy and cystectomy

Magers MJ, et al. - Forty-one subjects who attained a complete pathological response (ie, ypT0N0) disease following transurethral resection of bladder tumor (TURBT), neoadjuvant chemotherapy (NAC) and radical cystectomy (RC) with available clinical follow-up information were evaluated by the experts in order to represent a large tertiary care academic centre’s experience with subjects with ypT0N0 after NAC and RC with stress on morphological peculiarities present in the RC and clinical outcome. The recurrence-free survival at 1 and 5 years and the overall survival at 3 and 5 years was 97.4% and 93.5% and 94.2% and 88.6%, respectively. No subjects expired because of urothelial carcinoma. Stage allotted at TURBT was 1 pTa, 1 pT1, 38 pT2, and 1 pT3a. With most of these being squamous, 17 TURBTs exhibited variant histology. A scar, foreign body giant cell reaction, chronic inflammation within lamina propria and dystrophic calcifications were the most common morphological peculiarities present at RC, while other morphological peculiarities were less prevalent or absent. Hence, despite the stage or variant histology in the TURBT, ypT0N0 disease at RC portends an outstanding prognosis, and scar, foreign body giant cell reaction, chronic inflammation, and dystrophic calcifications were frequently seen.
