Clinical trajectory of patients with a worsening heart failure event and reduced ventricular ejection fraction

Carnicelli AP, Clare RM, Hofmann P, et al. - Presence of severe LV dysfunction was detected in more than half of patients with chronic heart failure with reduced ejection fraction (HFrEF) and worsening heart failure (WHF) in this study, and observations showed crucial disparities in comorbidities, HF therapies, and outcomes between those with left ventricular ejection fraction [LVEF] ≤25% and those with LVEF 26-35%.
This study included 2823 patients with chronic symptomatic HFrEF and WHF who were treated at Duke University between 1/2009 and 12/2018.
Younger age, more commonly male gender, and a lower cardiovascular comorbidity burden were present in patients with LVEF≤25% vs those with LVEF 26-35%.
Those with LVEF≤25% were less often on beta blockers (85.9% vs 90.5%) but more often received mineralocorticoid receptor antagonists (49.3% vs 41.1%) and implantable defibrillators (41.3% vs 28.2%).
Significantly higher hazards for death (HR 1.24), all-cause hospitalization (HR 1.21), and HF hospitalization (HR 1.25) through 5-years were observed in patients with LVEF≤25%.
