Clinical subtypes of medication overuse headache – Findings from a large cohort

Viana M, De Icco R, Allena M, et al. - Given that according to the medication overused, different subtypes of medication overuse headache (MOH) are listed by the International Classification of Headache Disorders, researchers examined if the distinct subtypes correspond to clinically distinguishable phenotypes in a large population. In this descriptive cross-sectional observational study, 660 patients with MOH in Europe and Latin America, were included as a part of the COMOESTAS project. Individuals overusing triptans (T-MOH), vs those overusing simple analgesic (A-MOH), were of higher mean age. T-MOH and individuals overusing ergotamines (E-MOH) reported longer duration of headache before chronification, 19.2 ± 11.9 years and 17.8 ± 11.7 years, respectively when compared with the A-MOH group (13.1 ± 10.9) and T-MOH reported longer duration of headache before chronification with respect to multiple drug classes (M-MOH, 14.9 ± 11.7). E-MOH group (33.6 ± 41.6) had significantly lower Migraine Disability Assessment (MIDAS) score, while MIDAS score was significantly lower in T-MOH group (56.8 ± 40.6) when compared with M-MOH (67.2 ± 62.5; P = 0.016 and P = 0.037, respectively). Patients overusing T with respect to other groups of patients had lower prevalence of depression and anxiety (χ2 = 10.953, P = .027 and χ2 = 25.725, P < 0.001, respectively).
