Clinical significance of granulomas in Crohn's disease: A systematic review and meta-analysis

Hong SW, et al. - Researchers sought to determine the clinical significance of epithelioid granuloma, one hallmark used to histologically diagnose Crohn's disease (CD), via performing a meta-analysis. They included 19 studies in this work. CD patients with granulomas had a higher proportion of ileocolonic disease, a higher proportion of upper gastrointestinal disease, a higher proportion of penetrating behavior, a higher prevalence of perianal disease, and a higher severity index at presentation. Further, they received biologics significantly more frequently than those without granulomas. A significant association of the presence of granulomas with CD-associated hospitalization, but not with CD-associated surgery was observed. Findings suggest it as indicative of a more aggressive phenotype of CD.
