Clinical research study implementation of case-finding strategies for heart failure and chronic obstructive pulmonary disease in the elderly with reduced exercise tolerance or dyspnoea: A cluster randomized trial

van Mourik Y, Rutten FH, Bertens LCM, et al. - Given that older individuals often remain undiagnosed with heart failure (HF) and chronic obstructive pulmonary disease (COPD), despite inhibited functionality and impaired health related to both disorders, researchers sought for the efficacy of a case-finding strategy of these disorders. They conducted a clustered randomized trial at 18 general practices from the vicinity of Utrecht, the Netherlands. Random allocation of 829 participants was done; 389 in the case-finding strategy group and 440 in the usual care group. They identified a number of new diagnoses of HF and COPD via the implementation of the case-finding strategy in primary care to multimorbid older people with dyspnoea or reduced exercise tolerance, however the strategy did not result in short-term improvement of health status compared to usual care.
