Clinical predictors of revision surgery for chronic rhinosinusitis with nasal polyposis within 5-year follow-up
American Journal of Otolaryngology - Head and Neck Medicine and Surgery Aug 11, 2020
Wu CL, Lee TJ, Huang CC, et al. - In the present study, the researchers sought to identify potential long-term predictors of revision sinus surgery for chronic rhinosinusitis with nasal polyposis (CRSwNP). Patients with bilateral CRSwNP who underwent primary endoscopic sinus surgery were prospectively recruited. Eighty-four qualified patients were identified, and after primary surgery, all experienced significant improvements in the visual analog scale. Data reported that the 5-year revision rate was 19.05%, and the meantime of revision surgery was 25.31 ± 17.11 months postoperatively. The independent risk factors for revision surgery were found to be nasal allergy and Lund-Mackay score (LMS). Findings suggested that patients with CRSwNP who have a concurrent nasal allergy and higher preoperative LMS may have an advanced disease status and may eventually be at high risk of revision surgery following long-term follow-up. In the future, an outcome-based staging system will help to improve the prognosis for CRSwNP.
