Modrall JG, et al. - Researchers sought to validate the three previously reported preoperative markers of BP response to renal artery stenting (RAS)—requirement for four or more antihypertensive medications, preoperative diastolic BP > 90 mm Hg, and preoperative clonidine use—using data from the Cardiovascular Outcomes in Renal Atherosclerotic Lesions (CORAL) trial, a multicenter randomized controlled trial. They assessed all patients randomized in the CORAL trial to RAS. Patients were categorized as BP responders or nonresponders to RAS based on American Heart Association guidelines. A postoperative BP < 160/90 mm Hg with a reduced number of antihypertensive medications or a reduction in diastolic BP to < 90 mm Hg with the same medications after RAS defined BP responders. Nonresponders comprised patients with stable or worsened BP. RAS was performed on 436 patients with a median age of 70 years (interquartile range [IQR], 63-76 years). As per multivariable logistic regression analysis, three independent markers of a positive BP response were: requirement for four or more medications, preoperative diastolic BP > 90 mm Hg, and preoperative clonidine use. Incremental increase in the percentage of patients with a positive BP response was observed as the number of markers per patient increased, based on the Cochran-Armitage test for trend. The findings validate the previously reported clinical markers of BP response in patients from the CORAL trial who underwent RAS.
