Clinical outcomes with beta-blocker use in patients with prior history of myocardial infarction

Jackevicius CA, Krumholz HM, Ross JS, et al. - In this cohort analysis utilizing administrative databases, researchers assessed the effectiveness of BB use in contemporary, stable patients with prior MI. They analyzed patients ≥ 65 years, alive on 4/1/2012 (index date) and had a hospital discharge diagnosis of MI within the previous 3 years. There were overall 33,811 patients with prior MI, of those, a BB was dispensed to 21,440 (63.4%). The study sample had a median age of 78 years and comprised 56% male. Findings revealed major cardiovascular events or mortality were not decreased in relation to BB use in those with prior MI within 3 years in this large, contemporary population-based observational analysis of older stable patients with previous MI. In the subgroup analysis by age, gender, MI timing, MI type, heart failure, and atrial fibrillation, no benefit was reported. Findings call for contemporary clinical trials assessing the use of BBs after MI.
