Clinical impact of lung ultrasound monitoring for diagnosis of ventilator associated pneumonia: A diagnostic randomized controlled trial

Journal of Critical Care Apr 18, 2020

Pradhan S, et al. - Researchers undertook this single-center diagnostic randomized controlled trial to determine if an earlier detection of ventilator-associated pneumonia (VAP) as well as improvement in patient outcome could be achieved with the use of lung-ultrasound monitoring. A combination of chest x-ray (CXR) and clinical findings was used to detect VAP among controls. A combination of lung-ultrasound and clinical findings was used for diagnosing VAP in the intervention group. Ventilator free days (VFD) was the primary outcome. The intervention group vs the control group was shown to have higher VFD. In terms of ICU length of stay, ventilator days, ICU mortality, delta SOFA and antibiotic duration, no disparities were evident between the groups. Overall, findings revealed improvement in patient outcome that was consequent to the use of lung-ultrasound monitoring for diagnosis of VAP vs the standard diagnostic strategy that depends on CXR.
