Clinical features and outcomes of asymptomatic pulmonary sarcoidosis: A comparative cohort study

Iriarte A, Rubio-Rivas M, Villalba N, et al. - Among patients with asymptomatic pulmonary sarcoidosis (APS) detected incidentally, researchers studied the clinical features as well as the outcomes and compared the findings with symptomatic non-Löfgren sarcoidosis (SnLS) patients. This study involved patients diagnosed with APS and followed prospectively from 1976 to 2018, recruited from a University hospital in Barcelona, Spain. The diagnosis of APS and of SnLS was received by 50 (13.6%) and 317 (86.4%) patients, respectively. Compared with SnLS, APS displayed earlier radiological stages, lesser impairment in lung function, extrapulmonary organ involvement as well as requirement for treatment. The most useful diagnostic instrument was endobronchial ultrasonography-guided transbronchial needle aspiration. Although benign presentation of this condition was seen, the progression to persistent disease was noted in around one third of patients but usually with mild clinical as well as functional impairment.
