Clinical features and outcomes of revascularization in very old patients with left main coronary artery disease

Sliman H, Jaffe R, Rubinshtein R, et al. - Researchers undertook this retrospective single-center study to determine clinical characteristics as well as outcomes of very old patients receiving revascularization due to left main coronary artery (LMCA) disease. Participants (n = 139) with at least 80 years of age, revascularized for coronary artery disease involving unprotected LMCA stenosis more than 50%, were examined for subsequent major adverse cardiovascular events (myocardial infarction, stroke, and all-cause death) and repeat revascularizations, as well as their link with revascularization procedure. Among study participants, findings revealed worse cardiovascular outcomes in relation to percutaneous coronary intervention (PCI) vs coronary artery bypass surgery, affected by a more severe and comorbid population selected for PCI. The identified additional independent outcome predictors were baseline disability, presentation with ST-segment elevation myocardial infarction, and distal LMCA bifurcation disease.
