Clinical factors associated with urinary tract cancer in individuals with Lynch syndrome

Wischhusen JW, et al. - In a cohort including 52,758 consecutively determined people undergoing Lynch syndrome testing at a commercial laboratory, researchers explored clinical factors related to urinary tract cancers (UTC; renal pelvis, ureter, bladder, and possibly kidney cancers) among Lynch syndrome carriers, by performing univariate analysis and multivariate logistic regression. The probability of having had UTC was found to be significantly more in Lynch syndrome carriers vs noncarriers. They observed the independent link of Lynch syndrome–related UTC with male gender, increased age, familial burden of UTC, and pathogenic EPCAM/MSH2 variants but not MLH1 variants, race, or history of other Lynch syndrome–associated malignancy. They noted the presence of ≥ 1 of the following features: male gender, EPCAM/MSH2 variants, or family history of UTC in 143 of 158 (90.5%) Lynch syndrome carriers with UTC; 1,236 of 1,251 (98.8%) Lynch syndrome carriers lacking all of these features had no history of UTC. Experts concluded that the recognition of Lynch syndrome carriers who are most likely to be at risk for UTC can reliably be done by taking specific clinical factors into account.
