Clinical effectiveness of cardiac noninvasive diagnostic testing in patients discharged from the emergency department for chest pain

Roifman I, Han L, Koh M, et al. - Among adults assessed for chest pain and discharged home from an emergency department in Ontario, Canada, researchers conducted this retrospective, population-based cohort analysis to determine if lower rates of myocardial infarction or cardiovascular death could be achieved by performing noninvasive diagnostic tests vs no testing. In order to decrease confounding between the testing and nontesting groups, they used propensity score matching. A cohort including 370,863 patients was assessed. In relation to noninvasive diagnostic testing after assessment for chest pain in the emergency department, a lower rate of the composite outcome of cardiovascular death or myocardial infarction was observed, which was driven by the high-risk subgroup. Considering risk-based testing for patients discharged from the emergency department for chest pain was recommended.
