Clinical assessment of aortic valve stenosis: Comparison between 4D flow MRI and transthoracic echocardiography

Adriaans BP, et al. - Via a prospective study of 20 individuals with different degrees of aortic stenosis (AS), the researchers intended to contrast four-dimensional phase-contrast magnetic resonance (4D PC-MR), two-dimensional (2D) PC-MR, and transthoracic echocardiography (TTE) for quantification of AS. Vpeak, Pmean, and aortic valve area (AVA) among TTE, 4D PC-MR, and 2D PC-MR were compared. In comparison to TTE, 4D PC-MR measured greater Vpeak and Pmean, however, a less critical AVA. When compared with TTE, unidirectional 2D PC-MR considerably minimized AS severity. Among 4D PC-MR and TTE, differences in Vpeak exhibited to be robustly associated with the eccentricity of the flow jet. Also, betterment in the concordance was seen via the use of 4D PC-MR, between Vpeak and AVA and between PGmean and AVA. Hence, among the various parameters of AS, 4D PC-MR improved the concordance and hence, could serve as a supplementary imaging technique following TTE. Also, in patients with discordant echocardiographic parameters, the potential value of 4D PC-MR should be discussed by further studies.
