Clinical and radiographic predictors of successful therapeutic bronchoscopy for the relief of malignant central airway obstruction

Giovacchini CX, Kessler ER, Merrick CM, et al. - Researchers explored clinical as well as radiographic predictors related to therapeutic bronchoscopy success in relieving malignant central airway obstruction (CAO). They performed a review of all cases of therapeutic bronchoscopy done for malignant CAO at their institution from January 2010 to February 2017. Achieving airway patency of > 50% defined therapeutic bronchoscopy success. They compared successful and unsuccessful groups in terms of patient demographics and baseline features, oncology history, degree of airway obstruction, procedural interventions, and complications. Distal patent airway visualized on CT scan and during bronchoscopy were identified as the predictors related to successful therapeutic bronchoscopy for malignant CAO. They found higher odds of success in non-smokers, and with reduced time from radiographic finding of CAO to intervention.
