Cholecystectomy vs cholecystostomy for the management of acute cholecystitis in elderly patients

Schlottmann F, et al. – This retrospective, population-based analysis compared post-procedural outcomes between cholecystectomy and cholecystostomy tube placement (CTP) in elderly patients (aged ≥ 65 years) using data from the National Inpatient Sample from 2000 to 2014. Researchers analyzed a total of 200,915 patients, of whom 7,516 underwent CTP and 193,399 underwent cholecystectomy. Findings revealed higher incidences of post-procedural morbidity and mortality, and longer post-procedure length of hospital stay among elderly patients with both acalculous and calculous acute cholecystitis managed with CTP vs cholecystectomy. Hence, the investigators recommend performing cholecystectomy on elderly patients with acute cholecystitis unless prohibitive surgical risks exist.
