Characterization of familial and sporadic migraine

Ravn J, et al. - In the present cross-sectional study, familial and sporadic migraine were differentiated with a focus on migraine characteristics, migraine severity, comorbidities, and treatment. From the Danish Headache Center, they analyzed 358 patients with familial migraine and 1,727 patients with sporadic migraine. Familial migraineurs vs sporadic migraineurs had more aphasic aura (41% vs 27%). Sporadic migraineurs vs familial migraineurs had more lifetime attacks ie, > 100 attacks (45% vs 70%) and prolonged attacks ie, lasting > 72 hours (5% vs 12%), and had a higher incidence of concussions (37% vs 41%). Individuals with and without inherited migraine were similar in headache characteristics, triptan response, and comorbidities, emphasizing to consider migraine as a homogenous disease. Further, patients with sporadic migraine with aura suffered more severe migraine, suggesting their exposure to stronger or multiple environmental factors and indicating that early intervention in migraine treatment could reduce the severity of migraine.
