Characterisation and diagnosis of ulcers in inpatient dermatology consultation services: A multi-centre study

Haynes D, Hammer P, Malachowski SJ, et al. - In this multi-centre retrospective review involving 272 patients (median age was 54 years) from four U.S. academic hospitals, researchers assessed the incidence of ulcer aetiology and diagnostic agreement between primary teams and consult specialists. Skin ulcers were considered the primary reason for admission in 49.3% of the patients. Investigators found that 62% ulcers were chronic and 49.6% were located on the lower extremities. The leading aetiologies were pyoderma gangrenosum, infection, and exogenous causes; after consultation, 12% remained diagnostically inconclusive. Predermatology and postdermatology consulting diagnostic agreements ranged from 0.104 to 0.553, indicating poor-modest agreement. This research shows the complexity of diagnosis and relative incidences of skin ulcers in the inpatient setting.
