Changes in renal function and predictors affecting renal outcome of living kidney donors: A nationwide prospective cohort study

Kim Y, Kim MJ, Lee YH, et al. - An important issue in living donor kidney transplantation is the safety of donors. Researchers herein analyzed data of 456 kidney donors registered to the Korean Organ Transplantation Registry from 2014 to 2016 with the aim to determine serial changes in renal function and predictors affecting the renal outcome of living kidney donors. At 2 years after kidney donation, chronic kidney disease (CKD) developed in 21.7% of the donors (99/456). Donors without CKD and those with CKD had annual estimated glomerular filtration rate (eGFR) changes after nephrectomy of 2.2 ml/min/1.73m2/year, and ‐0.4 ml/min/1.73m2/year, respectively. Overall, there appeared gradual increases in eGFR in kidney donors without incident CKD at 2 years after donation, whereas donors with CKD exhibited relatively constant eGFR. A risk factor of CKD comprised a low ratio of eGFR at discharge after nephrectomy to baseline.
