Cell-based functional IgE assays are superior to conventional allergy tests for shrimp allergy diagnosis

Wai CYY, Leung NYH, Leung ASY, et al. - In this study with 35 individuals with documented history of shrimp allergic reactions, researchers compared skin prick test (SPT), specific IgE (sIgE), basophil activation test (BAT) and IgE crosslinking-induced luciferase expression (EXiLE) test with respect to their diagnostic performance for shrimp allergy. Participants were categorized based on findings of double-blind placebo-controlled food challenge. Findings revealed a greater accuracy of BAT as a diagnostic marker for shrimp allergy compared with SPT and shrimp sIgE, while experts suggested EXiLE test based on IgE-crosslinking assay as a good alternative to BAT. In Chinese, tropomyosin may not represent the most crucial shrimp allergen, which warrants further inquiry to unveil other major allergens as well as diagnostic markers.
