Carotid endarterectomy and carotid artery stenting for patients with crescendo transient ischemic attacks: A systematic review

Fereydooni A, et al. - Since there does not exist any consensus on guidelines for medical management and the timing of revascularization in patients with crescendo transient ischemic attack, researchers undertook this systematic review to analyze data regarding the management of and timing of intervention in these patients. They explored PubMed (MEDLINE) and Google Scholar for all English-language articles published from January 1, 1985, to January 1, 2019. They did not include articles not involving analysis of patients with symptoms, not describing the timing of intervention, or mixed analysis of patients with stroke in evolution with patients with crescendo transient ischemic attack. A higher risk of stroke or death following carotid endarterectomy was seen in patients with crescendo transient ischemic attack vs patients with a single transient ischemic attack or stable stroke. The best management option for crescendo transient ischemic attack was optimal medical management as well as urgent carotid endarterectomy within 2 days of presentation. The increased embolic potential of bifurcation plaque was responsible for the preference given to surgical endarterectomy as a treatment option, whereas stenting was regarded as a choice for patients with contraindications for surgery.
