Cardiovascular risk factor reduction by community health workers in rural India: A cluster randomized trial

Joshi R, Agrawal T, Fathima F, et al. - In this open-label cluster-randomized trial, researchers determined the effect of community health worker (CHW)-based interventions in reducing risk factors for cardiovascular disease (CVD) in rural Indian households. A total of 1,172 and 1,140 households were randomized to intervention or control, respectively. Delivery of risk-reduction advice, as well as monitoring of risk factors during 6 household visits over 12 months by trained CHWs, was the strategy employed in the intervention group. Usual care was received by the households in the non-intervention group. An attenuation in systolic blood pressure (SBP) and adherence to prescribed BP-lowering drugs were assessed as the primary outcomes. Findings revealed that adherence to prescribed drugs was improved but SBP remained uninfluenced by a 12-month CHW-led intervention at the household level.
