Cardiac findings and hemodynamic changes associated with severe lower urinary tract obstruction in utero

Cohen J, et al. - Researchers sought to describe the hemodynamic impact of massive bladder distention, renal dysfunction, oligohydramnios, and lung hypoplasia observed in the fetus due to lower urinary tract obstruction (LUTO). They identified 26 fetuses with LUTO and ≥1 fetal echocardiogram; exclusion of one fetus was done to hypoplastic left heart syndrome. Compared to control fetuses, fetuses with LUTO exhibit left ventricular (LV) diastolic dysfunction alongside presenting with cardiomegaly, pericardial effusion, and ventricular hypertrophy, and seem to re-distribute cardiac output, which may contribute to the evolution of left heart hypoplasia.
