Can views of the proximal femur be reliably used to predict malrotation after femoral nail insertion? A cadaver validation study

Dubina AG, et al. - Sequential fluoroscopic images of 19 matched pairs of cadaver femora were collected of the proximal femur at 10° increments of internal and external rotation in order to examine the association between the size of the lesser trochanter visualized on an AP view of the hip and femoral rotation following femoral shaft fracture fixation. Rotation of the proximal femur followed a relatively linear association centered around the neutral rotation position. A 10% variation in the lesser trochanter size compared to nearly 7° of femoral rotation. Thus, the relationship between the size of the lesser trochanter visualized and the degree of femoral rotation following femoral shaft fracture fixation was nearly linear and sensitive to comparatively minute variations in rotation, making it possibly beneficial for evaluating malrotation following femoral shaft fracture fixation.
