Can digital breast tomosynthesis replace full-field digital mammography? A multireader, multicase study of wide-angle tomosynthesis

Georgian-Smith D, et al. - For breast cancer detection, researchers tested the premises if two-view wide-angle digital breast tomosynthesis (DBT) could replace full-field digital mammography (FFDM). In two reading sessions separated by more than 1 month, bilateral two-view FFDM and bilateral two-view wide-angle DBT images were viewed independently for breast cancer detection. Three hundred thirty patient cases were selected from a pool of 764 patients undergoing screening and diagnostic mammography. Investigators found that 29 of 31 readers, irrespective of breast density, performed better with DBT than FFDM. A statistically significant improvement was observed in readers' mean diagnostic accuracy with DBT. The detection accuracy of invasive cancers with DBT was significantly higher. According to this multireader multicase study, reader performance in breast cancer detection with wide-angle two-view DBT independent of FFDM was significantly higher, verifying DBT's robustness as a single view. Outcomes of perception studies in the vision sciences, however, support the inclusion of an overview image.
