C-reactive protein and immature-to-total neutrophil ratio have no utility in guiding lumbar puncture in suspected neonatal sepsis

Goldfinch CD, et al. - This study examined if inflammatory markers such as C-reactive protein (CRP) and immature-to-total neutrophil ratio (ITR) were speculative of meningitis or significant cerebrospinal fluid (CSF) pleocytosis and assisted in guiding the decision to perform a lumbar puncture (LP) inpatients of a single tertiary Neonatal Unit who were aged <6 months, who had LP performed between March 2011 and October 2014. The performance of CRP and ITR was poorly with regard to detecting infants with confirmed or probable meningitis. Data recommended placing more emphasis on clinical grounds and/or a positive blood culture and less on inflammatory or haematological markers in isolation while taking the decision to perform an LP.
