Bone microarchitecture and bone turnover in hepatic cirrhosis

Wakolbinger R, et al. - Considering the recent studies indicating low bone mineral density in cirrhosis, researchers sought to determine bone quality (three-dimensional microarchitecture) and bone strength in patients with cirrhosis and healthy controls via assessing their weight-bearing and non-weight-bearing bones. In this single-center study, thirty-two patients with recently diagnosed hepatic cirrhosis and 32 controls participated. Using high-resolution peripheral quantitative computed tomography, they assessed the parameters of bone microarchitecture. Outcomes revealed the deterioration of both trabecular and cortical microarchitecture in correlation to hepatic cirrhosis; this was noted irrespective of disease severity. All sites displayed diminishing of areal bone mineral density, a sign of generalized affection. Areal bone mineral density measurements are suggested as an appropriate tool for osteologic screening in patients with hepatic cirrhosis, regardless of its origin or disease severity.
