Biopsy-proven small-fiber neuropathy in primary Sjögren's syndrome: Neuropathic pain characteristics, autoantibody findings, and histopathological features

Birnbaum J, et al. - Experts evaluated neuropathic pain characteristics, autoantibody findings, and histopathological features among biopsy-proven small-fiber neuropathy (SFN) patients having primary Sjögren's syndrome (SS). They compared 23 consecutively evaluated SS patients with pure and mixed SFN vs those without it. They noted an increased frequency of male sex, decreased frequency of multiple antibodies, frequent treatment with opioid analgesics, and possibility to present with non-stocking-and-glove pain in association with SS SFN. In view of the fact that mechanisms targeting the dorsal root ganglia (DRG) may result in irreversible neuronal cell death, distinguishing between DRG and axonal injury was important. Altogether, clinical, autoantibody, and pathological features that can help determine mechanisms and treatment strategies were highlighted in the findings.
