Biliary atresia liver histopathological determinants of early post-Kasai outcome

Nguyen AHP, Pham YHT, Vu GH, et al. - Researchers performed a retrospective chart review of liver histologies in Kasai biliary atresia BA patients operated between January 2017 and July 2019 at their institution with the aim to determine histologic prognostic factors for biliary outcome. Based on a 3-month serum total bilirubin level of < 34 uM or mortality, categorization of patients with wedge liver biopsies and portal plate biopsies (n = 85) was done into unfavorable and favorable outcomes. Worse 1-year survival was recorded in correlation with total bilirubin levels > 34 uM. Overall findings revealed predictive value of advanced histologic findings of portal plate bile duct size of < 150 µm and severe hepatocellular damage for poor post-Kasai jaundice clearance and short-term survival outcome, irrespective of Kasai timing.
