Benefit of intravenous thrombolysis in acute ischemic stroke patients with high cerebral microbleed burden

Schlemm L, et al. - A multistep algorithm to model 90-day modified Rankin Scale scores in people with ≤ 10 vs > 10 cerebral microbleeds (CMBs) who do or do not receive intravenous thrombolysis (IVT) was made in order to evaluate the treatment impact of IVT in individuals with acute ischemic stroke and a high burden (>10) of CMBs. It was noticed that high CMB burden alters the treatment impact of IVT. In individuals with > 10 CMBs, IVT was related to higher mortality and a net utility loss in elderly people with severe strokes and longer treatment delays. Moreover, individuals with a higher-than-average pretest probability of > 10 CMB might benefit from MRI screening if it does not raise the treatment time.
