Atypical ductal hyperplasia is a multipotent precursor of breast carcinoma

Kader T, et al. - Given the lack of a known precursor to high grade (HG) cancer in the current model for breast cancer progression, which proposes independent “low grade (LG)-like” and “HG-like” pathways, researchers attempted to better describe breast cancer progression by applying low-coverage whole-genome sequencing to atypical ductal hyperplasia (ADH) with and without carcinoma. Based on the findings, they concluded that ADH can be a forerunner of HG breast cancer and that a similar ancestor lesion can give rise to LG and HG carcinomas. ADH may have a multipotent nature, able to progress to either LG or HG carcinoma, but LG ductal carcinoma in situ (DCIS) may be committed to a LG molecular pathway. The clinical significance of some ADH cases could be more than LG DCIS, as suggested by this multipotent nature, and therefore, biomarkers for personalization of management are needed in these cases.
