Association of timing of colostomy reversal with outcomes following Hartmann procedure for diverticulitis
JAMA Mar 27, 2019
Resio BJ, et al. - In this retrospective study of 1,660 patients from State Inpatient Databases, researchers examined the association of the timing of colostomy reversal with postoperative outcomes in patients who had the Hartmann procedure (end colostomy) for diverticulitis. For this investigation, patients were divided into early (45-110 days), middle (111-169 days), and late (≥170 days) reversal timing based on timing of colostomy reversal after index surgery. Data reported that the median time to reversal was 129 days. According to findings, less than one-third of patients undergo colostomy reversal within 1 year after end colostomy for diverticulitis, and reversal timing was related to socioeconomic disparities. Improved results were linked to the earlier reversal in selected patients with an uncomplicated course. Overall, the investigators concluded that colostomy reversal is safe in selected patients as early as 45 to 110 days following the Hartmann diverticulitis procedure.
