Association of thrombectomy with stroke outcomes among patient subgroups: Secondary analyses of the DEFUSE 3 randomized clinical trial

Lansberg MG, et al. - In the DEFUSE 3 randomized clinical trial, researchers identified outcomes in patients with acute ischemic stroke who may have a reduced thrombectomy effect, including those who are older, have milder symptoms or present late. Of the 296 patients enrolled in DEFUSE 3, 182 were randomized and included in the intention-to-treat analysis carried out in August 2017. Independent predictors of better functional outcome were younger age, lower baseline National Institutes of Health Stroke Scale score, and lower serum glucose level in the overall cohort. Investigators found that endovascular thrombectomy started within 16 hours of the last known well time in patients with salvageable tissue on perfusion imaging benefits patients with a broad range of clinical features. No significant interaction was found between this treatment effect and age, National Institutes of Health Stroke Scale score, time to randomization, imaging modality, or location of the arterial occlusion.
