Association of surgical intervention for adhesive small-bowel obstruction with the risk of recurrence

Behman R, et al. - Researchers investigated the association of operative intervention for adhesive small-bowel obstruction with long-term risk of recurrence in this propensity-matched study using population-level data of 27 904 patients. They noted significantly reduced (13.0% vs 21.3%) probability of recurrence in correlation with operative intervention for a patient’s first episode of adhesive small-bowel obstruction. With each episode, the risk of additional recurrences was noted to increase until surgical intervention, at which point the risk of subsequent recurrence decreased by approximately 50%. It seems that the long-term consequences of nonoperative management are not considered while advocating a trial of nonoperative management in the current standard for the management of adhesive small-bowel obstruction.
