Association of state Medicaid expansion with quality of care and outcomes for low-income patients hospitalized with acute myocardial infarction

Wadhera RK, et al. - In this retrospective cohort study involving 325,343 patients who had been hospitalized for acute myocardial infarction (AMI), researchers investigated if rates of uninsurance, quality of care, and outcomes changed in states that expanded Medicaid under the Patient Protection and Affordable Care Act vs nonexpansion states. This investigation was conducted at hospitals participating in National Cardiovascular Data Registry Acute Coronary Treatment and Intervention Outcomes Network Registry. Findings revealed that a significant attenuation in rates of uninsurance among patients hospitalized for AMI was observed in association with state Medicaid expansion. Among low-income adults in expansion vs nonexpansion states, no improvement was evident in the quality of care and outcomes. Compared with the past observations, hospital care for AMI was suggested to be possibly less sensitive to insurance.
