Association of seizure occurrence with aneurysm treatment modality in aneurysmal subarachnoid hemorrhage patients

Allen BB, et al. - The relationship between new-onset seizures after aneurysmal subarachnoid hemorrhage (aSAH) and aneurysm treatment modality, as well the link with initial clinical severity of aSAH and outcomes, were assessed in this retrospective cohort study. Following aSAH, infrequent occurrence of new-onset seizures was reported, and no significant difference was noted in the incidence after aneurysm clipping versys coilingl however, new seizures were shown to be more frequently associated with clipping than coiling in low-grade patients. Also, no occurrence of non-convulsive seizures was reported in low-grade patients treated with coiling. Previous work suggested better outcomes in coiled patients, for which these data may partly provide an explanation. These findings encourage physicians to have a lower threshold for continuous electroencephalogram utilization in low-grade patients suspected to have acute seizures after surgical clipping.
