Association of frailty and cognitive impairment with benefits of oral anticoagulation in patients with atrial fibrillation
American Heart Journal Feb 04, 2019
Madhavan M, et al. - Researchers examined the prevalence of clinically recognized cognitive impairment and frailty (according to American Geriatric Society Criteria) in the Outcomes Registry for Better Informed Care in Atrial Fibrillation (ORBIT AF). The investigators also assessed their association with adjusted outcomes via multivariable Cox regression. In this study sample consisting of 9,749 patients with AF, 293 and 575 were found to have cognitive impairment and frailty, respectively. Higher predicted risk for stroke and higher observed mortality were evident among those with cognitive impairment or frailty in AF, yet these patients were noted to be less frequently treated with oral anticoagulation (OAC). Despite this, patients with and without cognitive impairment or frailty exhibited similar benefits of OAC.
