Association of US News & World Report top ranking for gastroenterology and gastrointestinal operation with patient outcomes in abdominal procedures

JAMA Sep 26, 2019

Gambhir S, Daly S, Grigorian A, et al. - Researchers determined if US News & World Report (USNWR) top-ranked hospitals for gastroenterology and gastrointestinal surgical procedures vs nonranked hospitals are related to improvements in patient outcomes in common advanced laparoscopic abdominal operations. In this administrative database study, the Vizient database was utilized. Bariatric, colorectal, and hiatal hernia procedures were covered in abdominal operations. This study did not include surgeries on patients younger than 18 years, emergent cases, conversion cases, and patients with extreme severity of illness. At top-ranked hospitals vs at nonranked hospitals, the annual case volume was 397 vs 114, respectively. In-hospital mortality or serious morbidity did not vary significantly between top-ranked and nonranked hospitals. Higher mean expenses and longer mean lengths of stay were reported for advanced laparoscopic abdominal surgeries performed at top-ranked hospitals vs nonranked hospitals. Overall, experts concluded that despite the observed 3-fold higher annual volume of advanced laparoscopic abdominal operations for USNWR top-ranked hospitals vs nonranked hospitals among academic centers, there seemed no link between the volume and improved patient outcomes.
