Association between time to colonoscopy after positive fecal testing and colorectal cancer outcomes: A systematic review

Forbes N, Hilsden RJ, Martel M, et al. - Following a positive fecal screening test for colorectal cancer (CRC), a colonoscopy is required so researchers here assessed the extent to which time-to-colonoscopy is linked with CRC-related outcomes via a systematic review. An electronic search conducted through April 2020 yielded 1,612 initial studies; of these, 8 were incorporated in the systematic review, 5 of which described outcomes for fecal immunochemical testing (FIT). Although researchers could not conduct a meta-analysis, all studies indicated consistent trends between longer time delays and worse outcomes. Colonoscopy performed beyond 9 months from positive FIT vs within 1 month was significantly associated with a higher incidence of CRC. Based on outcomes, they emphasize not delaying colonoscopy for positive FIT beyond 9 months. Given that urgent referrals and surgical planning for CRC require an additional time, colonoscopy should ideally be conducted well in advance of 9 months after a positive FIT.
